Omeprazole (Inj: Na)
Indications/Uses
Cap: Short-term treatment of active duodenal & gastric ulcer, GERD & pathological hypersecretory conditions. Inj: Duodenal & gastric ulcers, reflux oesophagitis & in control of acid secretion in pathological hypersecretory conditions eg, Zollinger-Ellison syndrome.
Dosage/Direction for Use
Cap 1 cap once daily for 14 days. Inj Patient unable to take oral medication eg, in severely ill patient w/ either reflux oesophagitis, duodenal or gastric ulcer IV bolus/infusion once daily for up to 5 days. Zollinger-Ellison syndrome Initially 60 mg IV daily, adjust dose individually. Doses exceeding 60 mg daily should be given in divided doses bid.
Administration
Should be taken with food: Take immediately before a meal.
Contraindications
Hypersensitivity to omeprazole, or other PPIs (eg, lansoprazole, pantoprazole, rabeprazole). Cap: Trouble or pain in swallowing food, vomiting w/ blood or bloody or black stools. Not for immediate relief of heartburn symptoms or prevention of occasional heartburn. Inj: Pregnancy & lactation.
Special Precautions
Exclude possibility of stomach cancer/malignancy prior to/before treatment. Cap: Stomach ulcer, liver problems. Pregnancy & lactation. Childn <18 yr. Inj: Presence of increased gastric counts of bacteria in GIT. Increased risk of GI infection eg, Salmonella & Campylobacter. Hepatic impairment. Childn. Elderly.
Adverse Reactions
Diarrhea, abdominal pain, nausea, vomiting, constipation, flatulence; dizziness. Cap: Acid regurgitation; headache. Inj: Skin rash, pruritus & urticaria; paresthesia, lightheadedness & feeling faint; reversible mental confusion, agitation, depression & hallucinations in severely ill patients.
Drug Interactions
Cap: Reduced effects w/ rifampicin & St. John's wort. May increase effects of warfarin or other vit K antagonists, phenytoin, diazepam, digoxin, saquinavir, tacrolimus & cilostazol; increase gastric pH & reduce effects of ampicillin, Fe salts, dasatinib & antifungals eg, ketoconazole & itraconazole; increase effects of both omeprazole & voriconazole; decreased effects of clopidogrel & drugs for HIV treatment eg, atazanavir & nelfinavir. Inj: Delayed elimination, increased blood conc of diazepam, phenytoin & anticoagulants eg, warfarin. Reduced absorption of itraconazole & ketoconazole.
